Please be advised that North Florida Thyroid Center, the medical practice of Celeste Hart, MD, will permanently close effective May 18, 2026.  
Patients wishing to request transfer of medical records may contact the office by telephone at (850) 224-7154,  or by email at support@thyroidcenter.com, or via the website www.thyroidcenter.com.
Requests for records should include the patient's full name, date of birth, contact information and the name and address of the receiving physician or facility if appropriate.
