CLEVELAND PUBLIC LIBRARY NOTICE TO BIDDERS Sealed bids will be received by: Cleveland Public Library Louis Stokes Wing, 10th Floor Board Room 525 Superior Avenue NE Cleveland, Ohio 44114 Mailing address: 325 Superior Avenue NE Cleveland, Ohio 44114 for the following Project: Project: CPL Center for Media 1375 Euclid Avenue, Suite 460 Cleveland, OH 44115 in accordance with the Contract Documents prepared by: Payto Architects Inc. 1220 West Sixth Street, Suite 405 Cleveland, OH 44113 Phone: 216-241-6800 Contact: Jeffery Foster, AIA jfoster@paytoarchitects.com The project is the interior build out of a shell space in the IdeaCenter building at Playhouse square of approximately 5,700 sf of area for a video archive and processing lab. Work being procured by CPL includes: Finishes, various general trades, Electrical, HVAC and Fire protection. The work will be completed in conjunction with the Landlord's contractor who will be providing demolition, new wall framing with primed drywall, doors and hardware. Additional products, equipment, and services may be accepted as approved Substitutions only by written Addendum. If no Addendum is issued accepting the Proposed Substitution as an Approved Equal, the Proposed Substitution shall be considered rejected. Prevailing Wage Rates and Equal Employment Opportunity requirements are applicable to this Project. Subcontract agreements must comply with the Subcontract Form # C190-00 52 14 for Limited Scope Projects, as found on the Ohio Facilities Construction Commission website. Bidders are encouraged to be enrolled in and to be in good standing in a Drug Free Safety Program ("DFSP") approved by the Ohio Bureau of Workers' Compensation ("OBWC") prior to submitting a Bid and provide, on the Electronic Bid Form with its Bid, certain information relative to their enrollment in such a program; and, if awarded a Contract, shall comply with other DFSP criteria described in Section 1.3 of the General Conditions.
