PRIVATE PRACTICE CLOSURE
LEGAL NOTICE: NOTICE IS HEREBY GIVEN that Kurt LaRose, LCSW, Florida Licensed Clinical Social Worker, License No. SW9297, formerly practicing in offices in Tallahassee and also online through TALKIFUWANT.COM, has permanently ceased the operation of his clinical social work practice due to medical reasons. Clinical operations ended on MAY 3, 2026, following efforts during FEBRUARY, MARCH AND APRIL 2026 to determine whether the practice could remain in operation.
Former clients who wish to obtain electronic copies of their clinical records, or to request transfer of records to another licensed provider, may submit a written request via email at KURT@TALKIFUWANT.COM or by voicemail at (850) 545-2886. If you wish to mail your request send a release to: Kurt LaRose, 1701 E Harvest Cir, Perryville Mo. 63775. Requests should include the client's name, date of birth, and the name and contact information of the receiving provider, if applicable.
LaRose maintains all medical records in accordance with privacy laws. Medical records are retained in a secure HIPAA compliant server, for 7 years from the last date of service. As such, records are available upon written request during the 7 year retention period.
This notice is published once each week for FOUR (4) CONSECUTIVE WEEKS in The Tallahassee Democrat of general circulation in LEON COUNTY, FLORIDA, in compliance with Florida Rule 64B-9.00(3). Dated: 5/6/26 (1st submission).
