Doctor Departure Legal Notice:
In accordance with Ohio state regulations, notice is hereby given that Dr. Louis J. Schott of MidWest Eye Center, is leaving the practice located at 1060 Nimitzview Dr., Suite 105, Cincinnati, OH 45230 and 1275 North High St., Hillsboro, OH 45133. All patients may retrieve their medical records from MidWest Eye Center located at 1060 Nimitzview Dr., Suite 105, Cincinnati, OH 45230 and 1275 North High St., Hillsboro, OH 45133 or by calling 800-385-3937. Dr. Schott's last day at the practice will be June 1, 2026. It is important for existing patients to schedule an appointment within the next 30 days either with one of MidWest Eye Center's doctors or an outside eyecare doctor to ensure they are receiving ongoing care.
