NOTICE IS GIVEN YOU, Robert Supples, putative father, and unknown father of the minor, Leilani Alexander and TO ALL WHOM IT MAY CONCERN, that on July 24, 2025 petitions were filed under the Juvenile Court Act by KRISTEN A. STONE in this Court, and that in the courtroom of Judge Reginald C. Mathews C-2, Robert W. Depke Juvenile Justice Complex, 24647 North Milwaukee Avenue, Vernon Hills, Illinois, on June 16, 2026 at 9:00am or soon thereafter as this case may be heard, an adjudicatory hearing will be held upon the petition to have the child declared to be a ward of the Court under the Act. THIS COURT HAS AUTHORITY IN THIS CASE TO TAKE AWAY THE CUSTODY AND GUARDIANSHIP OF THE MINOR, TO TERMINATE YOUR PARENTAL RIGHTS, AND TO APPOINT A GUARDIAN WITH POWER TO CONSENT TO THE ADOPTION OF THE MINOR. YOU MAY LOSE ALL PARENTAL RIGHTS TO YOUR CHILD. Unless you appear, you will not be entitled to further written notices or publication notices of the proceedings in this case, including the filing of an amended petition or a motion to terminate parental rights.
Unless you appear at the hearing in person or through an attorney, an order of judgment by default will be entered against you for the relief asked in the petition, and thereafter any and all of your parental rights will be terminated and held for naught.
